What is Pyometra?

  • Pyometra is inflammation of the uterus.
  • Pyometra is caused by a bacterial infection supported by the hormone progesterone.
  • Most commonly, pyometra is seen in the diestrus stage of a bitche's cycle (the stage after estrous/heat).

Clinical Signs of Pyometra - WHAT to look for

General

  • Lethargy
  • Inappetence
  • Polyuria
  • Polydipsia
  • Vomiting

Open Cervix

  • General Signs From Above
  • Vulvar discharge which can include blood and pus

Closed Cervix

  • General Signs From Above
  • No discharge
  • Distended abdomen (due to the large uterus)

Severe

  • Intensified signs from the general list above
  • Shock
  • Sepsis

Diagnostics - Confirming That it is pyometra

  • Physical Examination
  • Ultrasound of Uterus
  • Radiograph of Uterus
  • CBC and Biochemistry - elevated WBCs to indicate infection and rule out any other conditions
  • Serum Progesterone - elevated p4 indicates hormones supporting pyometra
  • Uterine Culture and Sensitivity

Treatment - Getting Rid of the pyometra

  • Ovariohysterectomy with use of antibiotics
  • Medical Management - if future breeding is desired
    • Antibiotics
    • Prostaglandins
    • Antiprogestins

Common Questions

My dog/cat is pregnant now, will this affect the pregnancy?

  • Both treatment options of pyometra will end the pregnancy.

How will this affect future breeding?

  • If still intact your dog/cat will get pyometra again if no preventative measures are taken in the future for their cycles and pregnancies.
  • It should also be noted that the chances of getting pregnant are decreased by 50%.

My dog/cat is spayed, can she get pyometra?

  • If there is any uterine tissue left then there is a chance of pyometra. This is called uterine stump pyometra, but this is uncommon.

  • If breeding is desired - Medical management prior to and during heat and pregnancy
